Submit a Claim - Claim Instructions

If you are a Former Customer of CPAY, to claim your payment, you must submit a timely and valid Claim Form. The Claim Form must be submitted online no later than August 6, 2022, or, if mailed, postmarked no later than August 6, 2022.

How to File by Mail:

As part of your Notice, you were sent a Claim Form. You may also download a copy here. Read the instructions carefully, fill out the form, sign it, and mail it so that it is postmarked by August 6, 2022. To file by mail, you must send your completed Claim Form to the following address:

How to File Online:

Please locate your “Unique ID” that is printed on the top portion of the Claim Form you received in the mail. If you did not receive a Claim Form in the mail, you cannot file a claim online. You may file a claim by mail. Please see above for instructions.

After entering your Unique ID, you will be asked to provide your contact information. Make sure you read the form carefully, certify, and submit it no later than August 6, 2022.
